NOTICE TO CREDITORS AND CLAIMANTS

TO ALL THIS MAY CONCERN, please take notice that Gregory H. Kaake D.D.S., P.A., a Minnesota professional corporation subject to Chapter 302A and Chapter 319B of the Minnesota Statutes (the "Company"), is in the process of dissolving in accordance with and pursuant to Minn. Stat. S 302A.721.
1. On the 13th day of April, 2020, the Company filed a Notice of Intent to Dissolve with the office of the Minnesota Secretary of State in accordance with Minn. Stat. $ 302A.723.
2. Any and all claims against the Company must be presented in writing and described in reasonable detail with supporting justification to the following address:
