Four more clinics have been certified as “health-care homes” by the Minnesota Department of Health, including one in West Duluth.
A health-care home provides partnerships to improve health outcomes and quality of life for individuals with chronic health conditions and disabilities. They’re being developed as part of the health reform package the Minnesota Legislature passed in 2008.
To date, the state has certified 224 health-care homes. The latest four all are Essentia Health clinics, with clinics in Baxter, Brainerd and Moorhead certified as well as the West Duluth clinic.
