A Duluth nursing home is trying to reduce hospital readmissions.
Chris Jensen Health & Rehabilitation is working with Stratis Health, a nonprofit agency, and Essentia Health St. Mary’s Medical Center on the initiative, known as Improving Transitions of Care, a Chris Jensen news release said.
Currently, 11.36 percent of Chris Jensen residents admitted to St. Mary’s are admitted again within 30 days, said Jill Hess, administrator of the 170-bed nursing facility. That compares with a state average of 17.06 percent and a national average of 25.8 percent. But it could be better, Hess said.
“While we are pleased with our current statistics, we believe there is room for improvement,” she said in the news release.
The program’s goal is to reduce “avoidable” hospital readmissions by 20 percent, the news release said.
